A: You are correct. I think it is rude for the server to even ask, "Do you need change?" In my opinion, the proper thing is to state, "I'll bring your change right away." Then you have the opportunity to say, "No thanks". I give less of a tip if the waiter even asks that question, but I usually pay with a credit card anyhow.
